Headquartered in Woodland Hills, California, Health Net Inc. is a major publicly traded health care company. Health Net was originally incorporated in Delaware in 1990, but has since grown to serve 6.4 million members across 27 states with Medicare, Health Maintenance Organization (HMO), Point of Service (POS), and Preferred Provider Option (PPO) plans. Health Net also offers Dental and Vision plans, as well as term life insurance among its services. Overall, Health Net manages plans for individuals, as well as for corporations and the government. For their work, they were awarded an “Excellent” by the National Committee for Quality Assurance (NCQA), the highest accreditation possible. Not only that, they were also awarded highest marks in each of the five major categories, Access and Service, Qualified Providers, Staying Healthy, Getting Better, and Living with Illness.

SimpleChoice HSA - This plan has a $4,000 deductible, but covers most in-network costs once the deductible has been met. Most out-of network services are covered 50 percent, after the deductible has been met. Preventative care, such as mammograms and prostate checks cost $40, with the deductible waived. Note: this is an HSA, where money can be deposited tax-free to pay for medical expenses.
PPO SimpleChoice 15 - This PPO has a $1,500 deductible, but covers most in-network costs after the deductible has been met. Most out-of-network services are covered at 50 percent. Preventative care costs $15, with the deductible waived. However, maternity care is not covered. Medication is assessed a $250 deductible with a copay of $5 for generic drugs, and $35 to $50 for brand name or non-formulary drugs.
HMO 15 - As its name suggests, this is an HMO plan where most services -- office visits, immunizations, and preventative care -- cost $15. There is a $1,000 deductible for in-patient hospital services, but once covered, hospital care and maternity is covered in full. Medications have a $100 deductible, $15 to $30 for generic drugs, and $25 to $100 for other drugs. Premiums for this plan are $334 per month.
In addition to the features listed above, dental and vision coverage can be added for an additional $25 a month. Term life insurance is another monthly fee for $30,000 benefit coverage.
