HealthPartnersAbout HealthPartners Founded in 1957 as a cooperative, HealthPartners is the largest consumer governed non-profit healthcare organization in the nation, as well as the ninth largest employer in Minnesota. With more than 640,000 members, HealthPartners has a contracted network of 27,000 providers throughout Minnesota, Western Wisconsin, North and South Dakota, Iowa, and a nationwide network of 415,000 caregivers. According to a national survey sponsored by The National Business Coalition on Health called eValue8, HealthPartners is the top-rated plan in Minnesota, with one of the largest networks in the state. HealthPartners has “Excellent” accreditation with the National Committee for Quality Assurance (NCQA) for its point-of-service (POS), commercial health maintenance organization (HMO), and Medicare+Choice plans.
HealthPartners Plans HealthPartners offers open access networks, access to cost and care comparisons, and no referrals to visit specialists. It currently covers nearly one in four residents in the Twin Cities Metro area through a variety of high-quality and affordable individual plans, Medicare plans, and group plans. HealthPartners plans include: National Networks, Open Access Network, Tiered Network, Copay and Deductible, Self-Insured, Senior, Individual, Dental and Savings Accounts. HealthPartners Individual Plan offers eight deductible choices ranging from $150 to $5,000, with a limit of three deductibles per family each calendar year. The plan also features open access networks, no referrals, and travel coverage. Members may use service professionals, hospitals, and clinics; in-network and out-of-network care are subject to a separate deductible and co-insurance. The plan offers no vision or dental coverage and 80 percent coverage after deductible for emergency care, prescriptions, office visits, preventative health care, routine exams, inpatient and outpatient hospital care, and home health. HealthPartners Freedom Plans are Medicare Cost plans with several prescription drug options. Plan I, II, and III are available, with Plan I being the most affordable, Plan II the most popular, and Plan III the most comprehensive. HealthPartners Freedom Plus plans have an open access network that includes most doctors and hospitals throughout Minnesota. The HealthPartners Freedom II Plan includes: a $75 monthly premium and a $15 copay for office visits, no copay for routine exams, inpatient hospital copay of $150 per admit, and a $50 copay for emergency room care. Optional prescription drug plans include Option 1, no coverage; Option 2 Standard part D, with no annual deductible and a monthly payment of $19.98; Option 3 Enhanced Part D, no annual deductible or coverage gap, $10 copay for generics ($34 for brand name drugs), and a monthly payment of $67.66. HealthPartners Freedom Plan I Comprehensive Dental Plan offers members access to a network of dentists throughout Minnesota and 100 percent coverage on cleanings, routine examinations, and other preventive services. Regular and restorative care, special restorative care, and prosthetics are covered at 50 percent; members have access to a network of dentists throughout Minnesota. The monthly cost for HP Freedom Plan I Comprehensive Dental is $38.50 per month.
