In recent years, it has added vision packages, and is a strong leader in life insurance. Security Life has focused on providing full-service dental plans for more than 40 years. It prides itself on offering full-service solutions to the standard dental and vision insurance riders from other companies. Approximately 50 million premium holders reside in 44 states. Security Life Plans Security Life's dedicated dental and vision plans are provided by Stars Dental, which services PrimeStar Personal Dental Plans and GemStar Group Dental and Vision Plans. Dental plans are available for businesses and families; vision riders are economically scaled for small businesses. Employers can choose between partial plan payment or voluntary policies that are paid in full by the employees. Plan benefits are available as dental-only or dental-plus vision. Vision is a rider only. The dental program is available to small companies with one to 99 employees. Basic dental services include X-rays (bitewing every six months/diagnostic every three years), tooth extraction, fillings, and some oral surgeries. Preventive treatments such as routine six month exams and cleaning are covered. Major services will include such procedures as denture and bridge repairs, periodontal and endodontic work, and crown replacement. The three dental pricing levels are: Secure, Select, and Premier. All expenses are reimbursed based on a Reasonable and Customary (R&C) scale, which factors in competitors' rates by region. Rates are generally higher for those individuals over the age of 65. Plan participants are allowed to choose the licensed vision provider of their choice. Coinsurance levels automatically increase with each year of coverage. Rates are fixed for the first year, but may be raised in six month increments, following the initial signup period. Dental premiums are age- and region-based. Under the Competitor Smile dental plan, exclusions include orthodontics, loss or stolen appliances, full replacement of any dental product that can be repaired, and home treatments for fluoride plaque prevention. Vision care coverage is available to companies with two to 99 employees. The package includes 100 percent coverage of an annual eye exam that costs $50 or less, with no deductible. It provides complete coverage on contact lenses, glasses lenses, and frames every two years. For contacts, the payout is $100, single lenses ($50) and frames ($50) for a total of $100, bifocal lenses ($50) and frames ($50) for a total of $100, trifocal lenses ($90) and frames ($50) for a total of $140, and lenticular lenses ($100) and frames ($50) for a total of $150. Vision exclusions include but are not limited to prescription sunglasses, lost or broken glasses (either lenses or frames), and some surgical procedures. The plan does not cover frames priced over $75 or non-standard lenses (those that are greater than 61mm). Most major optical retailers are part of the vision network, along with approximately 32,000 independent eye specialists. Dental and vision plans are available in all states except Connecticut, Florida, Maine, Massachusetts, New Hampshire, New Jersey, New York, Rhode Island, South Dakota, and Vermont.
